All Hands-on Deck: Addressing Health and Social Needs Through Cross-Collaboration

Disrupting individual and systemic determinants of health requires a multi-layered approach that mobilizes systems from various disciplines and activates community-led change. The COVID-19 pandemic amplified the challenges associated with addressing these needs and the disproportionate ways in which marginalized communities are impacted. A collective impact model on the Westside of Atlanta committed to mobilizing resources and changing systems to achieve health equity established a rapid response to support the community during the pandemic. This rapid response was mobilized to support the community in accessing critical resources during the pandemic and assure sustained support. This session will address how to mobilize community partners to address health and social needs and how to foster a sustainable system of care that goes beyond the traditional health care model.

Learning Objectives:

  • Identify how collaborative efforts can foster a system of care that goes beyond the traditional health care model to identify and address social risk factors and social needs
  • Learn how to address complex challenges when aligning organizations from different sectors to drive change and focus on community rather an organizational plans and interests
